    Data Subject Rights Request

     

    General information and instructions

    The General Data Protection Regulation (GDPR) 2016 provides you, the Data Subject, with the right to access the data/information we hold about you, or to authorise someone to act on your behalf regarding your data. Please complete this form if you wish to access your data, or you want someone else to do so on your behalf. You will also need to provide proof of your identity. Your request will be processed within 30 calendar days upon receipt of a fully completed form and proof of identity. 

    The right to access information/data notes that you can:

    • Ask what information/data we are holding about you
    • Ask for modifying the held information/data about you if it is incorrect
    • Ask for restricting the use of or removing information/data we hold about you, provided that there are no legal reasons for us to process/retain the data/information.
    • Ask to have our company give the personal data you provided to us in a structured, commonly used and machine-readable format, and transfer that data to another data controller. 

    Proof of identity: We require proof of your identity before we can disclose personal data. Proof of your identity should include a copy of a document such as your birth certificate, passport, driving license. The documents should include your name, date of birth and current address. If you have changed your name, please supply relevant documents evidencing the change. If you are not the Data Subject, but you are acting on his/her behalf, in addition to the Data Subject proof of identity, please provide evidence of your authority to make the request on behalf of the Data Subject.

    The request form, if filled electronically, should be saved with your last name and the date (smith_20180402) and attached to an email and sent to the following address: dataprivacy@nch.com, or it can be sent via normal post to the local NCH subsidiary, as per the contact section on www.ncheurope.com or to:

    This request and any related correspondence will be retained to comply with legal obligation in our archives (also electronically) in accordance with NCH’s Record Retention Policy.
